About HFE

HFE encodes a cell-surface protein that helps regulate the body’s iron absorption by influencing how transferrin interacts with its receptor. Inherited changes in HFE can cause hereditary hemochromatosis, a disorder in which the body stores too much iron. HFE can also be altered in cancer, and inherited HFE changes may increase the risk of some cancers.

Met Not met Not assessed Not applicable

Variant-specific functional evidence is concordantly damaging across an orthogonal human-cell trafficking assay, a human-cell phenotype assay, and a homologous in vivo mouse model.

No variant-specific functional evidence supports preserved HFE function or normal splicing for BS3.

No VCEP covers HFE, so generic ACMG/AMP 2015 definitions govern.

PS4 was not met because no qualifying exact-variant affected-versus-control prevalence data support OR >5.0 with a confidence interval excluding 1.0.

PP5 and BP6 were restricted to exact-variant ClinVar expert-panel classifications; the audit reports none.

Population evidence supports BA1 because the all-comers gnomAD v4.1 allele frequency is 0.0569823, above 0.05.

BS1 is not assessed because the generic 1% default is insufficient for this ancestry-enriched, reduced-penetrance allele without a maximum-credible-frequency calculation.

BS2 and PM2 are not met: population homozygote counts do not establish the BS2 healthy-adult condition, and the allele is far above the PM2 rarity threshold.

PS1 StrongACMG_2015Met at strong strength: the identical p.Cys282Tyr change accounts for more than 80% of Caucasian hemochromatosis cases in the cited cohort summary. Details ▾Hide ▴
Reasoning

NM_000410.4:c.845G>A produces p.Cys282Tyr (C282Y), and this same amino-acid change is explicitly reported in affected individuals with HFE-related hemochromatosis. PMID:27518069 states that more than 80% of Caucasian hemochromatosis cases are related to C282Y homozygosity; PMID:10575540 reports C282Y in hemochromatosis probands with iron overload. These reports establish the same p.Cys282Tyr change as pathogenic at the protein level, with no evidence in the reviewed extracts that the observation is driven by a splicing effect.

Rule
Same amino acid change as a previously established pathogenic variant regardless of nucleotide change. ... Caveat: Beware of changes that impact splicing rather than at the amino acid/protein level.
Evidence considered
  • PMID:27518069, Introduction: more than 80% of hemochromatosis cases in Caucasians are related to homozygosity for HFE p.Cys282Tyr (c.845G>A).
  • PMID:10575540, Results/Table 2: HFE C282Y was identified in hemochromatosis probands with iron overload, including C282Y/G93R and C282Y/S65C genotypes.

PS3 StrongACMG_2015Met, strong: independent cellular and mouse studies showed loss of beta-2-microglobulin interaction, absent surface expression, and altered HFE-dependent phenotypes for C282Y. Details ▾Hide ▴
Reasoning

PS3 is met at strong strength. The exact HFE C282Y variant was tested in independent, biologically relevant functional systems with wild-type and comparator controls. In HEK293 cells, C282Y abolished beta-2-microglobulin interaction, eliminated detectable cell-surface expression, and caused intracellular/perinuclear retention, directly demonstrating defective HFE processing and trafficking (PMID:9162021). In a separate stable human neuroblastoma-cell system, C282Y altered HFE localization, ganglioside composition, sphingolipid-related gene expression, sphingosine-1-phosphate synthesis, morphology, and growth relative to wild-type HFE (PMID:21243428). A mouse model carrying the homologous C282Y substitution also showed abnormal HFE-dependent immune tolerance and cytotoxic T-cell responses (PMID:22531912). The concordant molecular, cellular, and in vivo findings support a damaging effect on the HFE gene product; the evidence is strong, although the studies were research assays rather than validated clinical diagnostic laboratory assays.

Rule
Well-established in vitro or in vivo functional studies supportive of a damaging effect on the gene or gene product. Note: Functional studies that have been validated and shown to be reproducible and robust in a clinical diagnostic laboratory setting are considered the most well-established.
Evidence considered
  • PMID:9162021, Results and Discussion, Figures 1–3: stable HEK293 cells expressing wild-type, C282Y-mutant, or H63D-mutant HLA-H; C282Y completely abrogated beta-2-microglobulin interaction, prevented detectable cell-surface expression, and disrupted intracellular protein trafficking.
  • PMID:21243428, Abstract and Results, Table 1 and Figures 2–7: stable SH-SY5Y cells expressing FLAG-tagged wild-type HFE, C282Y-HFE, or H63D-HFE showed altered localization, ganglioside composition, sphingolipid-gene expression, sphingosine-1-phosphate synthesis, morphology, and growth; five genes increased by >2-fold and two decreased relative to wild type.
  • PMID:22531912, Results and Discussion, pp. 853–858: mice carrying the homologous mHfe-C282→Y substitution showed abnormal selection and differentiation of mHFE-reactive CD8+ T cells and rejection of mHFE-expressing skin grafts.
  • The studies used wild-type HFE controls and, where reported, H63D comparator controls; the findings are concordant across direct protein trafficking, cellular phenotype, and an in vivo mouse model.

PP3 ModerateACMGMet at moderate strength: REVEL 0.872 exceeds the generic PP3 moderate threshold of 0.773 for missense variants. Details ▾Hide ▴
Reasoning

PP3 is applicable because NM_000410.4:c.845G>A is a missense variant producing p.(Cys282Tyr); under the generic missense path, REVEL is the sole applicable predictor and SpliceAI is not used. REVEL is 0.872, which meets the supplied generic PP3 moderate threshold of >= 0.773, calibrated by Pejaver et al. 2022 (PMID:36413997). BayesDel is not used because no generic ACMG-strength calibration is established.

Rule
Missense variant -> REVEL only. ... PP3 (moderate): >= 0.773 — ClinGen SVI REVEL calibration (Pejaver et al. 2022, Am J Hum Genet) (PMID:36413997)

  • Variant consequence: missense, NM_000410.4:c.845G>A, NP_000401.1:p.(C282Y).
  • Applicable generic path: missense variant -> REVEL only; the SpliceAI score is not used for PP3/BP4 in this missense situation.
  • REVEL score = 0.872 from the local REVEL lookup.
  • PP3 moderate threshold = >= 0.773, from the supplied ClinGen SVI REVEL calibration (Pejaver et al. 2022, PMID:36413997).

BA1 Stand aloneACMGMet: gnomAD v4.1 all-comers allele frequency is 0.0569823, exceeding the >=0.05 BA1 stand-alone threshold. Details ▾Hide ▴
Reasoning

Generic ACMG/AMP BA1 is met because NM_000410.4:c.845G>A (HFE p.Cys282Tyr) has an all-comers gnomAD v4.1 allele frequency of 0.0569823 (91,977/1,614,134 alleles), exceeding the published stand-alone threshold of >=0.05. The gnomAD v2.1 all-comers frequency is also 0.0337712 overall, with European non-Finnish frequency 0.057658; the available non-cancer subsets are concordant (v2.1 non-cancer exomes AF 0.0323788; v3.1 non-cancer genomes AF 0.0369491). No VCEP-specific population source restriction governs this case, so the generic all-comers default was used.

Rule
Allele frequency is above 5% in Exome Sequencing Project, 1000 Genomes or Exome Aggregation Consortium.
Evidence considered
  • gnomAD v4.1 all-comers: 91,977/1,614,134 alleles, AF=0.0569823, homozygotes=3,399; European non-Finnish AF=0.0710229.

PS4 ACMG_2015Not met: no affected-versus-control variant counts permit the required PS4 OR greater than 5.0 with a 95% CI excluding 1.0. Details ▾Hide ▴
Reasoning

No qualifying PS4 case-control comparison establishes that NM_000410.4:c.845G>A is significantly more prevalent in affected individuals than controls. Beutler et al. (PMID:11812557) screened 41,038 health-appraisal participants and found 152 homozygotes; the abstract states that common symptoms were no more prevalent among homozygotes than controls and that only one homozygote had signs suggesting hemochromatosis. This is not a case-versus-control variant-prevalence table, so an OR and 95% CI cannot be computed from the reported data. The literature_pass packet listed multiple papers under PS4, but its ai_classified_papers contained no paper classified as PS4; the reviewed exact-variant papers report clinical cases, cohort context, or functional findings without the required affected-versus-control variant counts.

Rule
The prevalence of the variant in affected individuals is significantly increased compared to the prevalence in controls. ... Relative risk (RR) or odds ratio (OR), as obtained from case-control studies, is >5.0 and the confidence interval around the estimate of RR or OR does not include 1.0.
Evidence considered
  • Beutler et al., PMID:11812557, abstract: 41,038 individuals were screened; 152 homozygotes were identified; common symptoms were no more prevalent among homozygotes than controls, and only one homozygote had signs and symptoms suggesting hemochromatosis. The publication is not present as a source_registry key, so it is reported in this narrative but not placed in evidence_refs.
  • PMID:9162021 reports C282Y in 83% of hereditary-hemochromatosis patient DNAs and provides functional cell evidence, but no matched unaffected-control variant count or OR/95% CI.
  • PMID:27518069 studied 206 patients referred for suspected type 1 hemochromatosis and describes C282Y heterozygotes with additional rare HFE variants; it does not provide a case-control prevalence comparison for C282Y.
  • The literature_pass ai_classified_papers list contained no paper with PS4 in its criteria field; the packet's PS4 index was therefore checked without treating its unverified index as evidence.

PM2 ACMGNot met: gnomAD v4.1 all-comers AF is 0.0569823 with 3,399 homozygotes, far above the <=0.0001 PM2 threshold. Details ▾Hide ▴
Reasoning

PM2 is not met because the variant is common in population controls rather than absent or extremely rare: gnomAD v4.1 all-comers AF=0.0569823 with 3,399 homozygotes, gnomAD v2.1 all-comers AF=0.0337712 with 276 homozygotes, and the non-cancer subsets also show AF=0.0323788 in v2.1 exomes and AF=0.0369491 in v3.1 genomes.

Rule

PP5 ACMG_2015Not met: ClinVar reports zero exact-variant expert-panel submissions, so ordinary pathogenic submissions cannot trigger PP5. Details ▾Hide ▴
Reasoning

The exact variant has no ClinVar expert-panel submission: the ClinVar audit reports zero expert-panel submissions, while the available Pathogenic assertions are ordinary laboratory or single-submitter records. The reported pathogenic literature and aggregate ClinVar label therefore do not satisfy the restricted PP5 trigger.

Rule
Reputable source recently reports variant as pathogenic, but the evidence is not available to the laboratory to perform an independent evaluation.
Evidence considered
  • ClinVar exact-match audit for NM_000410.4:c.845G>A reports expert_panel_submissions = 0; available Pathogenic records are single-submitter or laboratory submissions, not expert-panel classifications.
  • The literature_pass packet's PP5 papers were treated only as pathogenic reports and were not used to override the required exact-variant expert-panel condition.

BS2 ACMG_2015Not met: gnomAD v4.1 reports 3,399 homozygotes, but population records do not establish healthy adults with full early-age penetrance. Details ▾Hide ▴
Reasoning

The variant is observed in many gnomAD homozygotes, including 3,399 in gnomAD v4.1 all-comers, 276 in gnomAD v2.1 all-comers, 223 in gnomAD v2.1 non-cancer exomes, and 143 in gnomAD v3.1 non-cancer genomes. However, these aggregate population records do not establish that the individuals were healthy adults with full penetrance expected at an early age, as required by BS2; therefore the observed homozygotes do not meet BS2.

Rule
Observed in a healthy adult individual for a recessive (homozygous), dominant (heterozygous), or X-linked (hemizygous) disorder, with full penetrance expected at an early age.
Evidence considered
  • gnomAD v4.1 all-comers: homozygotes=3,399; gnomAD v2.1 all-comers: homozygotes=276.
  • gnomAD v2.1 non-cancer exomes: homozygotes=223; gnomAD v3.1 non-cancer genomes: homozygotes=143.
  • The available aggregate gnomAD records do not provide the healthy-adult status, age, clinical evaluation, or full early-age penetrance required to use these homozygotes for BS2.

BS3 ACMG_2015Not met: C282Y abolished beta-2-microglobulin interaction and surface expression in HEK293 cells rather than showing no damaging effect on HFE function. Details ▾Hide ▴
Reasoning

BS3 is not met. The available variant-specific functional studies do not show no damaging effect; instead, they show defective HFE beta-2-microglobulin interaction, loss of cell-surface expression, abnormal intracellular trafficking, and altered cellular and in vivo phenotypes for C282Y (PMID:9162021, PMID:21243428, PMID:22531912). Therefore the evidence is directionally opposite to the BS3 requirement, and no well-established benign functional assay result was identified.

Rule
Well-established in vitro or in vivo functional studies show no damaging effect on protein function or splicing.
Evidence considered
  • PMID:9162021, Results and Discussion, Figures 1–3: C282Y abolished beta-2-microglobulin interaction, prevented detectable cell-surface expression, and disrupted intracellular protein trafficking, which is inconsistent with a no-damaging-effect result.
  • PMID:21243428, Abstract and Results, Table 1 and Figures 2–7: C282Y caused altered HFE localization and multiple sphingolipid-related cellular phenotypes relative to wild-type HFE, not preserved function.
  • PMID:22531912, Results and Discussion, pp. 853–858: the homologous mouse C282Y model showed abnormal HFE-dependent immune tolerance and cytotoxic T-cell responses, not a benign functional phenotype.

BP2 ACMG_2015Not met: the reported phase is trans, whereas BP2 requires trans only for fully penetrant dominant disorders or cis in any inheritance pattern. Details ▾Hide ▴
Reasoning

The available allelic observations report C282Y in trans with G93R or S65C, not in cis. BP2 permits a trans observation only for a fully penetrant dominant gene/disorder, or a cis observation in any inheritance pattern. The available HFE-related hemochromatosis evidence describes a recessive context and provides no cis observation; therefore BP2 is not met.

Rule
Observed in trans with a pathogenic variant for a fully penetrant dominant gene/disorder or observed in cis with a pathogenic variant in any inheritance pattern.
Evidence considered
  • Barton et al. 1999, PMID:10575540: C282Y was reported on a separate chromosome from G93R in Proband 2 and separately from S65C in another affected proband; no cis configuration was reported.

BP4 ACMGNot met: REVEL 0.872 is above the generic BP4 supporting cutoff of 0.29 for missense variants. Details ▾Hide ▴
Reasoning

BP4 is applicable because NM_000410.4:c.845G>A is a missense variant, but the generic missense path permits REVEL only. REVEL is 0.872, which is above the highest supplied BP4 cutoff of <= 0.29 and therefore does not support a benign computational effect. The SpliceAI max delta of 0.075 is not considered for BP4 because splice prediction is not the applicable path for a missense variant; BayesDel is not usable because no generic ACMG-strength calibration is established.

Rule
Missense variant -> REVEL only. ... BP4 (supporting): <= 0.29 — ClinGen SVI REVEL calibration (Pejaver et al. 2022, Am J Hum Genet) (PMID:36413997)
Evidence considered
  • Variant consequence: missense, NM_000410.4:c.845G>A, NP_000401.1:p.(C282Y).
  • Applicable generic path: missense variant -> REVEL only; SpliceAI is not used for BP4 in this missense situation.
  • REVEL score = 0.872 from the local REVEL lookup.
  • BP4 supporting threshold = <= 0.29, from the supplied ClinGen SVI REVEL calibration (Pejaver et al. 2022, PMID:36413997); REVEL 0.872 does not meet it.
  • SpliceAI max delta = 0.075 is not evidence for BP4 because this is a missense variant and the rule requires the REVEL path.
  • BayesDel has no generic ACMG-strength calibration and is not used for PP3/BP4.

PS2 ACMG_2015Not assessed: no documented proband phenotype, parental maternity and paternity testing, or family-history assessment is available. Details ▾Hide ▴
Reasoning

PS2 cannot be assessed. The case materials do not document a disease-affected proband with this variant, absence of family history, or confirmed maternity and paternity. A ClinVar submission records origin as de novo, but it has no validated literature trail and is not sufficient to establish the required parental testing or clinical context. The governing definition requires both maternity and paternity confirmation; no such evidence is provided.

Rule
De novo (both maternity and paternity confirmed) in a patient with the disease and no family history.
Evidence considered
  • ClinVar variation 9, NM_000410.4(HFE):c.845G>A (p.Cys282Tyr): one submission lists origin as de novo, but the submission has no validated PubMed trail; this does not document confirmed maternity and paternity or the proband's disease and family-history context.
  • No case-level report in the reviewed evidence documents both parental testing results for this proband.
Still missing

PM1 ACMG_2015Not assessed: Cys282 is functionally important, but no qualifying HFE hotspot/domain and no benign-variation exclusion are established in the supplied evidence. Details ▾Hide ▴
Reasoning

The variant is p.Cys282Tyr at residue 282. vcep_materials.json has no HFE domain_tables entry, and the reviewed literature documents functional importance of Cys282 but does not establish a qualifying mutational hotspot or critical, well-established functional domain together with absence of benign variation. Therefore PM1 cannot be assessed from the supplied evidence without re-deriving or independently validating domain and benign-variation data.

Rule
Located in a mutational hot spot and/or critical and well-established functional domain (e.g. active site of an enzyme) without benign variation.
Evidence considered
  • vcep_materials.json: domain_tables is empty for HFE, so no authoritative VCEP domain list is available.
  • PMID:9162021, Abstract/Results/Discussion: C282Y completely abrogates beta-2-microglobulin interaction and disrupts intracellular protein trafficking, documenting functional importance of the residue but not a qualifying PM1 domain/hotspot or absence of benign variation.

PM3 ACMG_2015Not assessed: two affected probands had C282Y in trans with G93R or S65C, but the partner variants are not independently established as pathogenic. Details ▾Hide ▴
Reasoning

HFE-related hemochromatosis is represented as a recessive disorder in the available case evidence. PMID:10575540 reports two affected probands with C282Y paired with another HFE variant: C282Y/G93R and C282Y/S65C. In Proband 2, RT-PCR, subcloning, and sequencing demonstrated that C282Y and G93R were on separate chromosomes, establishing trans phase; the paper likewise reports separate chromosomes for the C282Y/S65C observation. However, the available evidence does not independently establish G93R or S65C as pathogenic variants under the governing PM3 definition. Therefore, the phase and affected-proband observations are relevant but insufficient to assess PM3 as met.

Rule
For recessive disorders, detected in trans with a pathogenic variant...Note: This requires testing of parents (or offspring) to determine phase.
Evidence considered
  • Barton et al. 1999, PMID:10575540: Alabama hemochromatosis probands included C282Y/G93R and C282Y/S65C genotypes; family studies and RT-PCR with subcloning and sequencing established that the paired variants occurred on separate chromosomes.
  • The same source documents hemochromatosis with iron overload in the reported probands, but the reviewed source does not independently classify G93R or S65C as pathogenic.
Still missing
  • Independent pathogenic classification or equivalent evidence for the G93R and S65C partner variants is not available in the reviewed evidence.

PM6 ACMG_2015Not assessed: a database de novo label lacks a case report documenting the proband, family history, and unconfirmed parental origin. Details ▾Hide ▴
Reasoning

PM6 cannot be assessed. The materials do not provide a case-level report establishing that this variant arose de novo without parental confirmation. Although one ClinVar submission records origin as de novo, it has no validated literature trail and does not supply the proband phenotype, family history, or the parental testing information needed to distinguish an assumed de novo event from an unsupported database annotation.

Rule
Assumed de novo, but without confirmation of paternity and maternity.
Evidence considered
  • ClinVar variation 9, NM_000410.4(HFE):c.845G>A (p.Cys282Tyr): one submission lists origin as de novo, but the submission has no validated PubMed trail and provides no case-level parental-origin details.
  • No reviewed publication establishes an assumed de novo occurrence of this variant in a disease-affected proband.
Still missing
  • Primary case report for the de novo observation
  • Documented disease phenotype and diagnostic context for the proband
  • Documented absence of relevant family history
  • Clarification of whether either or both parents were tested

PP1 ACMG_2015Not assessed: no affected-relative transmission count is reported; the available study shows variant phase, not cosegregation across multiple affected family members. Details ▾Hide ▴
Reasoning

PP1 cannot be assessed. No family pedigree or count of affected and unaffected relatives carrying or lacking NM_000410.4:c.845G>A is provided. PMID:10575540 reports family studies and molecular phasing in HFE probands with C282Y plus another HFE variant, including separation of C282Y from G93R or S65C, but this is phase information rather than cosegregation with disease in multiple affected family members. Therefore no PP1 evidence level can be assigned.

Rule
Co-segregation with disease in multiple affected family members in a gene definitively known to cause the disease.
Evidence considered
  • Barton et al. 1999, PMID:10575540: the reported Alabama cohort included a C282Y/G93R proband and a C282Y/S65C proband; family studies and RT-PCR/subcloning showed the variants were on separate chromosomes, which is phase information and not affected-family cosegregation.
  • No pedigree or informative affected-relative meiosis count for NM_000410.4:c.845G>A is available in the reviewed materials.
Still missing
  • Pedigree with phenotype status for relatives
  • Genotypes for affected and unaffected relatives

PP2 ACMG_2015Not assessed: missense HFE disease variants are documented, but the supplied evidence lacks a gene-level benign-missense rate assessment. Details ▾Hide ▴
Reasoning

The reviewed evidence confirms that missense HFE variants occur in hemochromatosis, but it does not provide a gene-level assessment demonstrating both a low rate of benign missense variation and that missense variants are a common disease mechanism under a defined dataset or threshold. PP2 is therefore not assessed.

Rule
Missense variant in a gene that has a low rate of benign missense variation and where missense variants are a common mechanism of disease.
Evidence considered
  • PMID:10575540: multiple HFE missense variants, including C282Y, G93R, and S65C, are reported in hemochromatosis probands.
  • PMID:27518069: rare HFE variants are evaluated in patients with suspected type 1 hemochromatosis, but the extract does not quantify the gene-wide benign missense rate.
Still missing
  • A systematic HFE gene-level analysis establishing low benign missense variation is unavailable.

PP4 ACMG_2015Not assessed: the present case lacks phenotype or family-history details demonstrating a highly specific hereditary-hemochromatosis presentation. Details ▾Hide ▴
Reasoning

The available case materials do not provide a patient phenotype or family history for this case that can be judged highly specific for hereditary hemochromatosis with a single genetic etiology. Variant reports in the literature cannot substitute for the phenotype of the present case.

Rule
Patient’s phenotype or family history is highly specific for a disease with a single genetic etiology.
Evidence considered
  • The case summary identifies HFE c.845G>A/p.C282Y but supplies no patient-specific phenotype or family-history description.
Still missing
  • Detailed patient phenotype, biochemical findings, diagnostic assignment, and relevant family history are needed to assess specificity.
BS1 ACMGNot assessed: AF 0.0569823 exceeds 1%, but no prevalence-, penetrance-, and allelic-heterogeneity-based maximum credible frequency was supplied. Details ▾Hide ▴
Reasoning

The observed frequency is well above the generic BS1 default (gnomAD v4.1 all-comers AF 0.0569823; European non-Finnish AF 0.0710229; gnomAD v2.1 AF 0.0337712), but the default 1% threshold cannot be applied alone here. HFE p.Cys282Tyr is a common ancestry-enriched founder allele, and HFE-related hemochromatosis has age-dependent and reduced clinical penetrance; a maximum-credible-frequency calculation using disorder prevalence, penetrance, and allelic heterogeneity was not supplied. BS1 therefore remains not assessed rather than being assigned from the generic threshold alone.

Rule
Allele frequency is greater than expected for the disorder.
Evidence considered
  • gnomAD v4.1 all-comers AF=0.0569823 and European non-Finnish AF=0.0710229; gnomAD v2.1 all-comers AF=0.0337712 and European non-Finnish AF=0.057658.
  • The generic BS1 default supplied for this case is >=0.01, but the case instructions require a maximum-credible-frequency assessment when reduced penetrance, late onset, or ancestry-specific founder enrichment may permit a pathogenic allele above 1%.
  • No per-disorder maximum-credible-frequency calculation with prevalence, penetrance, and allelic-heterogeneity inputs was available for this adjudication.
Still missing
  • A maximum-credible-allele-frequency calculation is needed, with explicit disease prevalence, penetrance, and allelic heterogeneity inputs, before BS1 can be applied to this reduced-penetrance founder allele.

BS4 ACMG_2015Not assessed: no affected non-carriers or pedigree-based non-segregation analysis is documented for this variant. Details ▾Hide ▴
Reasoning

BS4 cannot be assessed. The materials contain no affected family members who were tested negative for NM_000410.4:c.845G>A and no pedigree-based non-segregation analysis. The absence of reported segregation data is not evidence of lack of segregation, so BS4 is not met and cannot be assigned without documented affected non-carriers and appropriate consideration of phenotype heterogeneity or phenocopies.

Rule
Lack of segregation in affected members of a family.
Evidence considered
  • No affected relative documented in the reviewed evidence is reported to lack NM_000410.4:c.845G>A.
  • No family-level non-segregation analysis is available; lack of such data cannot be treated as lack of segregation.
Still missing
  • Affected relatives tested for the variant
  • Reliable phenotype assessment of those relatives
  • Pedigree-based documentation of variant absence in affected members
  • Assessment of phenocopies and other pathogenic variants in the family
BP1 ACMG_2015Not assessed: supplied HFE reports document missense disease variants but do not establish that truncating variants are the primary disease mechanism. Details ▾Hide ▴
Reasoning

The variant is missense, satisfying BP1's variant-type premise, but the supplied evidence does not establish that HFE-related disease is caused primarily by truncating variants. The reviewed HFE disease reports instead document multiple missense variants, which is insufficient for a definitive gene-wide mechanism conclusion.

Rule
Missense variant in a gene for which primarily truncating variants are known to cause disease.
Evidence considered
  • PMID:10575540: affected hemochromatosis probands include HFE missense variants C282Y, G93R, and S65C.
  • PMID:27518069: the study evaluates rare HFE variants in patients with suspected hemochromatosis and does not establish a primarily truncating mechanism.
Still missing
  • A gene-wide HFE disease-mechanism assessment comparing truncating and missense pathogenic variants is unavailable.

Reasoning

No evidence in the case materials establishes that this variant was found in a case with an alternate molecular basis for disease. The presence of other HFE variants in some published cases is not evidence for an alternate molecular basis in this case.

Rule
Variant found in a case with an alternate molecular basis for disease.

PVS1 N/ANot applicable: PVS1 requires a null variant (nonsense, frameshift, canonical splice, start-loss or stop-loss), and this is a missense variant. Details ▾Hide ▴
Reasoning

PVS1 (ClinGen SVI PVS1 recommendations, PMC6185798, pvs1_generic_framework) applies only to null variant classes — nonsense, frameshift, canonical ±1/2 splice-consensus, translation-initiation-codon loss, and stop-loss variants — each expected to trigger nonsense-mediated decay or otherwise abolish normal protein function. NM_000410.4:c.845G>A in HFE is a missense substitution predicted to produce NP_000401.1:p.(Cys282Tyr). As a result, no null-variant mechanism (nonsense-mediated decay, truncation, or canonical splice disruption) is triggered by this variant class, so PVS1's structural prerequisite is not met by this variant class. No VCEP criteria specification covers PVS1 for HFE, so the generic ACMG/AMP 2015 definition applies. This determination was made deterministically from the variant's consequence class, prior to LLM adjudication.

PM4 N/ANot applicable: PM4 requires a protein-length change, and this is a missense variant. Details ▾Hide ▴
Reasoning

PM4 (Richards et al. 2015, PMID:25741868) applies to protein length changes: in-frame insertions/deletions in a non-repetitive region, or stop-loss variants that extend the protein past the reference stop codon. NM_000410.4:c.845G>A in HFE is a missense substitution predicted to produce NP_000401.1:p.(Cys282Tyr). As a result, no change in protein length occurs, so there is nothing for this criterion to evaluate, so PM4's structural prerequisite is not met by this variant class. BP7 N/ANot applicable: BP7 requires a synonymous variant, and this is a missense variant. Details ▾Hide ▴
Reasoning

BP7 (Richards et al. 2015, PMID:25741868) is defined for a synonymous (silent) variant for which splicing prediction algorithms predict no impact to the splice consensus sequence and the nucleotide is not highly conserved. NM_000410.4:c.845G>A in HFE is a missense substitution predicted to produce NP_000401.1:p.(Cys282Tyr). As a result, the encoded protein sequence is altered, so the criterion's silent-variant premise does not hold regardless of any splicing prediction, so BP7's structural prerequisite is not met by this variant class. How the predictions were used

Evidence considered
  • Variant consequence: missense, NM_000410.4:c.845G>A, NP_000401.1:p.(C282Y).
