About SMAD4

SMAD4 encodes a protein that helps transmit TGF-beta and bone morphogenetic protein signals to the nucleus, where it controls genes involved in cell growth and tissue development. It acts as a tumor suppressor and is associated with juvenile polyposis syndrome and hereditary hemorrhagic telangiectasia. Loss or alteration of SMAD4 is especially linked to pancreatic cancer and can also occur in colorectal and lung cancers.

LYFE Sciences is an AI system, and it can make mistakes. Criteria may be applied incorrectly, sources may be misread, and a confident-looking classification can still be wrong. Double-check every criterion and its underlying evidence before relying on any call.

The SMAD4 VCEP is listed without released criteria, so the supplied generic ACMG/AMP rules govern.

No qualifying case-control prevalence estimate, patient-specific phenotype, alternate molecular basis, or exact-variant expert-panel classification was identified for these five criteria.

The variant is missense, so PP3 and BP4 use the REVEL path only; the same REVEL result is not double-counted between criteria.

REVEL 0.955 supports PP3 at strong strength and does not support BP4.

PS3 StrongACMG_2015Met, strong: two variant-specific recombinant-protein assays consistently found that SMAD4 R361H disrupted or completely abolished Smad2-Smad4 complex formation. Details ▾Hide ▴
Reasoning

PS3 is met at strong strength. Two independent, variant-specific biochemical studies evaluated SMAD4 p.Arg361His using recombinant protein constructs and appropriate comparator mutations/controls. Wu et al. assessed purified SMAD4 and SMAD2 domains by size-exclusion chromatography and related biophysical analyses, reporting that R361H disrupted Smad2-Smad4 heterocomplex formation while evaluating protein folding and solution properties. Shi et al. used a recombinant phosphorylated Smad2-Smad4 MH2-domain gel-filtration assay and reported that R361H completely abolished complex formation, whereas a comparator SMAD4 mutation had no detectable effect. These orthogonal, biologically relevant assays directly test the SMAD4 interaction required for TGF-beta signaling and consistently demonstrate loss of function for the exact variant. The evidence is strong functional support, although the bundle does not document validation in a clinical diagnostic laboratory setting.

Rule
Well-established in vitro or in vivo functional studies supportive of a damaging effect on the gene or gene product.
Evidence considered
  • Wu et al. 2001, PMID:11274206: recombinant SMAD4 residues 251-552 and SMAD2 residues 241-467 were purified and assessed by size-exclusion chromatography and related biochemical analyses; the exact SMAD4 R361H mutation disrupted heterocomplex formation, with protein folding and solution properties also evaluated.
  • Shi et al. 2002, PMID:11779503: recombinant phosphorylated SMAD2 MH2 domain and SMAD4 MH2 domain were analyzed by gel filtration; the exact SMAD4 R361H mutation completely abolished Smad2-Smad4 complex formation, while comparator SMAD4 D537H had no detectable effect.
  • The two studies provide concordant, biologically relevant loss-of-function results in independent biochemical assay implementations, but do not document clinical diagnostic-laboratory validation or a formal assay reproducibility study.

PM2 SupportingACMG_2015Met at supporting strength: the variant is absent from gnomAD v2.1 and v4.1, corresponding to frequency 0, below the PM2 threshold of <=0.0001. Details ▾Hide ▴
Reasoning

PM2 is met at supporting strength. NM_005359.6:c.1082G>A is absent from gnomAD v2.1 and gnomAD v4.1, and the available non-cancer subset entries also report absence. This satisfies the generic PM2 population requirement; under the supplied ClinGen SVI default, PM2 is supporting and the <=0.0001 threshold is met by an observed allele count of zero.

Rule
Absent from controls (or at extremely low frequency if recessive) in Exome Sequencing Project, 1000 Genomes or Exome Aggregation Consortium.
Evidence considered
  • gnomAD v2.1 reports the variant as absent, with no observed alternate allele.
  • gnomAD v4.1 reports the variant as absent, with no observed alternate allele.
  • The available GNOMAD_V2_1_NON_CANCER and GNOMAD_V3_1_NON_CANCER entries also report absence; no released SMAD4 VCEP rule requires a different population source.

PP3 StrongACMGMet, strong: missense REVEL score 0.955 meets the PP3 strong threshold of >=0.932. Details ▾Hide ▴
Reasoning

The variant is a missense substitution, NM_005359.6:c.1082G>A (p.Arg361His), so the governing generic rule selects the REVEL path only; SpliceAI and BayesDel are not used for PP3/BP4 here. REVEL is 0.955, meeting the generic PP3 strong threshold of >= 0.932 from the ClinGen SVI REVEL calibration (Pejaver et al. 2022, PMID:36413997).

Rule
PP3: Multiple lines of computational evidence support a deleterious effect on the gene or gene product (conservation, evolutionary, splicing impact, etc.). ... Missense variant -> REVEL only. ... PP3 (strong): >= 0.932 — ClinGen SVI REVEL calibration (Pejaver et al. 2022, Am J Hum Genet) (PMID:36413997)
Evidence considered
  • Variant consequence is missense: NM_005359.6:c.1082G>A produces NP_005350.1:p.(Arg361His) in exon 9.
  • Local REVEL predictor lookup reports a score of 0.955 for chr18:51065549 G>A.
  • The supplied ClinGen SVI REVEL calibration sets PP3 strong at >= 0.932; Pejaver et al. 2022, PMID:36413997.
  • SpliceAI max delta 0.079 and BayesDel 0.578234 were not counted because the missense rule selects REVEL only and BayesDel has no generic ACMG-strength calibration.

PP5 ACMG_2015Not met: the exact-variant ClinVar record has 0 expert-panel submissions, and its Pathogenic/Likely pathogenic assertions are clinical-laboratory submissions. Details ▾Hide ▴
Reasoning

PP5 requires a reputable recent report of the exact variant as pathogenic when the supporting evidence is unavailable for independent evaluation. The exact-variant ClinVar record is Pathogenic/Likely pathogenic, but the audit documents 0 expert-panel submissions and only clinical-laboratory submissions; under the specified generic rule, these assertions do not qualify for PP5.

Rule
Reputable source recently reports variant as pathogenic, but the evidence is not available to the laboratory to perform an independent evaluation.
Evidence considered
  • ClinVar exact match NM_005359.6(SMAD4):c.1082G>A (p.Arg361His) reports Pathogenic/Likely pathogenic submissions, but the submission audit records expert_panel_submissions = 0 and identifies the assertions as clinical testing submissions rather than an expert-panel classification.

BA1 ACMG_2015Not met: the variant is absent from gnomAD v2.1 and v4.1, so its frequency is not >=0.05, the generic BA1 threshold. Details ▾Hide ▴
Reasoning

BA1 is not met. NM_005359.6:c.1082G>A (SMAD4 p.Arg361His) was absent from gnomAD v2.1 and gnomAD v4.1, as well as the available non-cancer subsets; therefore no allele frequency or grpmax FAF approaches the generic BA1 threshold of >=0.05. No population-frequency evidence supports a stand-alone benign call.

Rule
Allele frequency is above 5% in Exome Sequencing Project, 1000 Genomes or Exome Aggregation Consortium.
Evidence considered
  • gnomAD v2.1 reports the variant as absent, with no allele frequency, allele count, or grpmax FAF reported.
  • gnomAD v4.1 reports the variant as absent, with no allele frequency, allele count, or grpmax FAF reported.
  • The available GNOMAD_V2_1_NON_CANCER and GNOMAD_V3_1_NON_CANCER entries also report absence; no governing VCEP frequency source is released for SMAD4, so generic rules apply.
Sources
BS1 ACMG_2015Not met: gnomAD v2.1 and v4.1 show no observed allele, so the frequency is below the generic BS1 threshold of 0.01. Details ▾Hide ▴
Reasoning

BS1 is not met. The variant is absent from gnomAD v2.1 and v4.1, including the available non-cancer subset entries, so there is no observed population frequency or grpmax FAF >=0.01. The absence provides no evidence that the allele frequency is greater than expected for SMAD4-related disease.

Rule
Allele frequency is greater than expected for disorder.
Evidence considered
  • gnomAD v2.1 reports the variant as absent, with no allele frequency, allele count, or grpmax FAF reported.
  • gnomAD v4.1 reports the variant as absent, with no allele frequency, allele count, or grpmax FAF reported.
  • The available GNOMAD_V2_1_NON_CANCER and GNOMAD_V3_1_NON_CANCER entries also report absence; no ancestry-specific elevation, founder signal, prevalence estimate, penetrance estimate, or maximum-credible-frequency calculation is needed to explain an elevated frequency because no elevated frequency is observed.
Sources
BS3 ACMG_2015Not met: two exact-variant assays reported disrupted or completely abolished Smad2-Smad4 complex formation rather than preserved protein function. Details ▾Hide ▴
Reasoning

BS3 is not met because the available variant-specific functional evidence does not show a lack of damaging effect. Instead, two recombinant-protein studies directly evaluated SMAD4 p.Arg361His and reported disrupted or completely abolished Smad2-Smad4 heterocomplex formation, a biologically relevant function of the SMAD4 protein. No exact-variant assay in the reviewed evidence demonstrates normal protein function or normal splicing.

Rule
Well-established in vitro or in vivo functional studies show no damaging effect on protein function or splicing.
Evidence considered
  • Wu et al. 2001, PMID:11274206: exact SMAD4 R361H disrupted formation of the Smad2-Smad4 heterocomplex in recombinant-protein assays.
  • Shi et al. 2002, PMID:11779503: exact SMAD4 R361H completely abolished formation of the Smad2-Smad4 complex in a recombinant phosphorylated MH2-domain assay.
  • The reviewed exact-variant functional evidence is damaging rather than benign and therefore contradicts the no-damaging-effect requirement for BS3.

BP4 ACMGNot met: missense REVEL score 0.955 exceeds the BP4 supporting threshold of <=0.29. Details ▾Hide ▴
Reasoning

The variant is a missense substitution, so BP4 must be assessed using REVEL only. REVEL is 0.955, which is above rather than at or below the strongest generic BP4 threshold of <= 0.016 and also exceeds the supporting threshold of <= 0.29; therefore BP4 is not met. The SpliceAI max delta of 0.079 is not considered because splice prediction is not the applicable path for a missense variant.

Rule
BP4: Multiple lines of computational evidence suggest no impact on gene or gene product (conservation, evolutionary, splicing impact, etc) ... Missense variant -> REVEL only. ... BP4 (supporting): <= 0.29 — ClinGen SVI REVEL calibration (Pejaver et al.
  • The supplied ClinGen SVI REVEL calibration sets BP4 supporting at <= 0.29, moderate at <= 0.183, and strong at <= 0.016; Pejaver et al. 2022, PMID:36413997.
  • SpliceAI max delta 0.079 was not used because the governing rule selects REVEL for missense variants.

BP6 ACMG_2015Not met: the exact-variant ClinVar audit has no benign expert-panel classification and instead contains Pathogenic/Likely pathogenic laboratory submissions. Details ▾Hide ▴
Reasoning

BP6 requires a reputable recent report of the exact variant as benign when the supporting evidence is unavailable for independent evaluation. The exact-variant ClinVar audit contains no benign expert-panel classification and instead records Pathogenic/Likely pathogenic clinical-laboratory submissions, so the specified generic PP5/BP6 rule does not permit BP6.

Rule
Reputable source recently reports variant as benign, but the evidence is not available to the laboratory to perform an independent evaluation.
Evidence considered
  • ClinVar exact match NM_005359.6(SMAD4):c.1082G>A (p.Arg361His) has no expert-panel submissions and no benign or likely-benign expert-panel classification; recorded laboratory submissions are Pathogenic or Likely pathogenic.

PS1 ACMG_2015Not assessed: no previously established pathogenic Arg361His caused by a different nucleotide change was documented for codon 361. Details ▾Hide ▴
Reasoning

The variant is p.Arg361His (c.1082G>A), and the reviewed literature documents this exact amino-acid change, but no evidence identifies a previously established pathogenic p.Arg361His caused by a different nucleotide change. The available ClinVar record is for the same c.1082G>A allele, not an alternate nucleotide change at codon 361. The caveat about splice effects is not limiting here because the variant is annotated as missense and SpliceAI's maximum delta score is 0.079.

Rule
Same amino acid change as a previously established pathogenic variant regardless of nucleotide change. ... Caveat: Beware of changes that impact splicing rather than at the amino acid/protein level.
Evidence considered
  • ClinVar identifies the exact queried allele as NM_005359.6:c.1082G>A (p.Arg361His), but does not provide a different nucleotide change producing p.Arg361His.
  • Wu et al. 2001 (PMID:11274206) reports tumor-derived SMAD4 R361H, and Shi et al. 2002 (PMID:11779503) reports SMAD4 R361H, but neither establishes an alternate-nucleotide Arg361His allele for PS1.
  • SpliceAI max delta score is 0.079, supporting evaluation at the protein level rather than a splice-impact caveat.
Still missing
  • A validated pathogenic SMAD4 Arg361His allele caused by a different nucleotide substitution is required.

PS2 ACMG_2015Not assessed: no documented maternity-and-paternity confirmation or case-specific de novo result is available for SMAD4 c.1082G>A. Details ▾Hide ▴
Reasoning

No case-specific evidence documents that NM_005359.6:c.1082G>A (SMAD4 p.Arg361His) is de novo, and no confirmed maternity and paternity results are provided. The available literature and case records do not establish both parental relationships, absence of the variant in both parents, a disease phenotype in the proband, or absence of family history. Therefore, PS2 cannot be assessed.

Rule
De novo (both maternity and paternity confirmed) in a patient with the disease and no family history.
Evidence considered
  • The case evidence identifies SMAD4 NM_005359.6:c.1082G>A (p.Arg361His), but contains no parental testing, de novo status, maternity confirmation, paternity confirmation, or family-history result for this proband.
  • The reviewed variant-specific literature describes functional studies or tumor observations, not a confirmed de novo occurrence in this proband.
Still missing
  • Documented variant testing in both biological parents
  • Confirmed maternity and paternity
  • Proband disease phenotype and family-history assessment
PS4 ACMG_2015Not assessed: affected-case reports lack control counts or an odds ratio and 95% confidence interval required to show OR >5.0 with a CI excluding 1.0. Details ▾Hide ▴
Reasoning

The governing PS4 definition requires a significantly increased prevalence in affected individuals versus controls; Note 1 specifies OR or RR >5.0 with a confidence interval excluding 1.0. The reviewed literature identifies SMAD4 p.Arg361His in affected or tumor-derived reports, including PMID:15014009 Table 2 and PMID:11274206, but provides no matched control count, case/control frequency comparison, OR/RR, or 95% confidence interval for this exact variant. The papers listed as PS4 candidates in literature_pass.json were checked through the available extracts and literature metadata; they do not establish the required case-control comparison. Because the required comparison cannot be calculated, PS4 is not assessed rather than inferred from repeated case reports.

Rule
The prevalence of the variant in affected individuals is significantly increased compared to the prevalence in controls. ... Note 1: Relative risk (RR) or odds ratio (OR), as obtained from case-control studies, is >5.0 and the confidence interval around the estimate of RR or OR does not include 1.0.
Evidence considered
  • PMID:15014009, Table 2 lists MADH4 codon 361 CGC-to-CAC (p.R361H) in colorectal cancer reports, but the paper provides no control comparison or exact-variant prevalence estimate.
  • PMID:11274206 and PMID:11779503 study the tumor-derived SMAD4 R361H mutation functionally; neither reports affected-versus-control counts or an OR/RR with a confidence interval.
  • The PS4 candidate literature entries in literature_pass.json were reviewed; repeated reports in affected individuals are not sufficient for the ACMG/AMP PS4 case-control threshold without control data.
Still missing
  • Exact-variant case and matched-control counts or frequencies are unavailable.
  • No exact-variant OR/RR and 95% confidence interval are reported.

PM1 ACMG_2015Not assessed: Arg361 is within the reported MH2 mutational cluster (330-370), but absence of benign variation in that region was not established. Details ▾Hide ▴
Reasoning

SMAD4 p.Arg361His lies in the MH2-domain mutational cluster region reported as codons 330-370, and biochemical studies place R361H in the loop-helix region important for Smad2-Smad4 interaction. However, the generic PM1 definition also requires that the hotspot or critical domain be without benign variation, and the available evidence does not establish that condition across the relevant region. The SMAD4 VCEP record has no released criteria and vcep_materials.json has no domain_tables entry, so no authoritative gene-specific domain list or PM1 rule resolves this missing requirement.

Rule
Located in a mutational hot spot and/or critical and well-established functional domain (e.g. active site of an enzyme) without benign variation.
Evidence considered
  • Iacobuzio-Donahue et al. 2004 (PMID:15014009) places codon 361 within the SMAD4/MADH4 MH2-domain mutation cluster region at codons 330-370 and describes the region as a loop/helix structure important for intermolecular interactions.
  • Shi et al. 2002 (PMID:11779503) reports that Smad4 R361H in the loop-helix region completely abolished Smad2-Smad4 complex formation.
  • The CSPEC record is present but has no released criteria; vcep_materials.json reports domain_tables as an empty object.
Still missing
  • Evidence that the relevant SMAD4 MH2-domain hotspot or critical region lacks benign variation is needed.
Sources
PM5 ACMG_2015Not assessed: no established pathogenic missense substitution different from Arg361His was documented at residue 361. Details ▾Hide ▴
Reasoning

The queried variant changes Arg361 to His. The reviewed evidence documents R361H itself, but no different missense substitution at residue 361 with an established pathogenic classification was identified. Reported D351H and V370D variants are at different residues and therefore do not satisfy PM5.

Rule
Novel missense change at an amino acid residue where a different missense change determined to be pathogenic has been seen before. ... Caveat: Beware of changes that impact splicing rather than at the amino acid/protein level.
Evidence considered
  • The exact queried variant is Arg361His; the collected ClinVar record concerns the same allele and does not identify a different missense change at Arg361.
  • The literature reports D351H and V370D as other SMAD4 tumor-derived substitutions, but these are at residues 351 and 370, not residue 361.
  • No same-residue comparator satisfying the PM5 requirement was present in the reviewed evidence.
Still missing
  • A validated pathogenic SMAD4 missense variant other than Arg361His at residue 361 is required.

PM6 ACMG_2015Not assessed: no case-specific evidence shows an assumed de novo SMAD4 c.1082G>A result without parental confirmation. Details ▾Hide ▴
Reasoning

No case-specific evidence documents an assumed de novo occurrence of NM_005359.6:c.1082G>A (SMAD4 p.Arg361His). The available records do not report the variant in the proband relative to either parent, nor do they establish whether parental testing was absent or incomplete. Therefore, PM6 cannot be assessed.

Rule
Assumed de novo, but without confirmation of paternity and maternity.
Evidence considered
  • The case evidence contains no parental genotypes or case report stating that SMAD4 c.1082G>A was assumed de novo.
  • The reviewed publications do not provide a qualifying de novo observation for this exact variant in the case under review.
Still missing
  • Case-specific de novo assertion
  • Proband and parental variant results
  • Documentation of whether maternity and paternity were unconfirmed
PP1 ACMG_2015Not assessed: no affected-family-member genotypes or meioses demonstrate co-segregation of SMAD4 c.1082G>A with disease. Details ▾Hide ▴
Reasoning

No case-specific pedigree or testing data document co-segregation of NM_005359.6:c.1082G>A (SMAD4 p.Arg361His) with disease in affected family members. The literature includes reports of other SMAD4 variants and general SMAD4-related disease cohorts, but those data do not establish segregation of this exact variant in this family. Therefore, PP1 cannot be assessed.

Rule
Co-segregation with disease in multiple affected family members in a gene definitively known to cause the disease. ... May be used as stronger evidence with increasing segregation data.
Evidence considered
  • The case evidence provides no family pedigree, affected-relative testing, segregation count, or informative meioses for SMAD4 c.1082G>A.
  • The reviewed literature contains observations of other SMAD4 variants or unsegregated case-series data, which cannot be used as co-segregation evidence for this exact variant.
Still missing
  • Pedigree with affected-status definitions
  • Genotypes for affected relatives
  • Number of informative meioses and co-segregating relatives
PP2 ACMG_2015Not assessed: SMAD4 loss of function is supported, but no qualifying low benign-missense-rate evidence was supplied. Details ▾Hide ▴
Reasoning

The evidence supports SMAD4 loss of function as a disease mechanism, but it does not establish both PP2 requirements: a low rate of benign missense variation and missense variants being a common disease mechanism. No gene-level benign-missense-rate metric or qualifying comparative dataset was supplied for SMAD4.

Rule
Missense variant in a gene that has a low rate of benign missense variation and where missense variants are a common mechanism of disease.
Evidence considered
  • The gene-level context supports SMAD4 loss of function as a germline disease mechanism, but does not quantify benign missense variation or establish the PP2 gene-level combination.
  • The queried variant is missense and is clinically classified in ClinVar, but an individual variant classification cannot substitute for the PP2 gene-level requirements.
Still missing
  • A validated SMAD4 benign-missense-rate assessment is required.
  • Evidence that missense variation is a common SMAD4 disease mechanism under the PP2 rule is required.

PP4 ACMG_2015Not assessed: the case lacks a documented patient phenotype or family history demonstrating a highly specific single-gene disease presentation. Details ▾Hide ▴
Reasoning

The governing PP4 definition requires the patient's phenotype or family history to be highly specific for a disease with a single genetic etiology. The case evidence identifies SMAD4 and the associated juvenile polyposis/hereditary hemorrhagic telangiectasia disease context, but does not provide this patient's phenotype, family history, or a sufficiently specific clinical presentation. General disease descriptions and literature-level syndrome statements cannot substitute for the patient's phenotype.

Rule
Patient’s phenotype or family history is highly specific for a disease with a single genetic etiology.
Evidence considered
  • The case identifies the SMAD4-associated juvenile polyposis/hereditary hemorrhagic telangiectasia syndrome context, but no patient-specific clinical phenotype or family history is supplied.
Still missing
  • Patient phenotype and family history are not documented in the available evidence.
BS2 ACMG_2015Not assessed: gnomAD absence does not document the required healthy-adult genotype observation for BS2. Details ▾Hide ▴
Reasoning

BS2 is not assessed. The population evidence shows absence from gnomAD v2.1 and v4.1 but does not document an individual-level observation of this variant in a healthy adult, a homozygote, a heterozygote, or a hemizygote. A population-database absence cannot establish the healthy-adult observation required for BS2.

Rule
BS2
Evidence considered
  • gnomAD v2.1 and v4.1 report the variant as absent and provide no individual-level healthy-adult genotype observation.
  • The available evidence does not establish a healthy adult carrying the variant or provide the age, phenotype, zygosity, and penetrance context required to adjudicate BS2.
Still missing
  • Individual-level evidence showing this variant in a healthy adult with the genotype relevant to the disorder's inheritance and expected age-dependent penetrance.
Sources
BS4 ACMG_2015Not assessed: no tested affected relatives are documented as lacking SMAD4 c.1082G>A, so non-segregation cannot be evaluated. Details ▾Hide ▴
Reasoning

No case-specific evidence documents affected family members who were tested and found not to carry NM_005359.6:c.1082G>A (SMAD4 p.Arg361His). The available records do not provide a pedigree, affected-relative genotypes, or phenotype adjudication sufficient to evaluate non-segregation, and no conclusion can be drawn from the absence of family data. Therefore, BS4 cannot be assessed.

Rule
Lack of segregation in affected members of a family. ... Caveat: The presence of phenocopies for common phenotypes (i.e. cancer, epilepsy) can mimic lack of segregation among affected individuals.
Evidence considered
  • The case evidence contains no affected-relative genotypes or documented lack of segregation for SMAD4 c.1082G>A.
  • No family-level phenotype and alternative-causation assessment is available to distinguish true non-segregation from phenocopy or another pathogenic variant.
Still missing
  • Affected-relative testing results

  • Assessment for phenocopies or another pathogenic variant in the family
BP1 ACMG_2015Not assessed: loss-of-function evidence exists, but a primarily truncating SMAD4 disease spectrum was not established. Details ▾Hide ▴
Reasoning

The available gene-context evidence supports loss of function as a SMAD4 disease mechanism, but it does not demonstrate that primarily truncating variants, rather than missense variants, are known to cause disease. The supplied evidence lacks a systematic variant-spectrum comparison sufficient for BP1.

Rule
Missense variant in a gene for which primarily truncating variants are known to cause disease.
Evidence considered
  • The gene-level context supports SMAD4 loss of function as a disease mechanism but does not establish that truncating variants primarily cause disease.
  • Iacobuzio-Donahue et al. 2004 describes SMAD4 missense mutations and a missense mutational cluster, which does not establish the BP1 primarily-truncating pattern.
Still missing
  • A systematic SMAD4 disease-variant spectrum demonstrating that disease is primarily caused by truncating variants is required.

BP2 ACMG_2015Not assessed: no pathogenic partner variant or parent/offspring phase result establishes the required trans or cis relationship for NM_005359.6:c.1082G>A. Details ▾Hide ▴
Reasoning

BP2 cannot be assessed from the available evidence. Although the CSPEC disease framework identifies SMAD4-related juvenile polyposis/hereditary hemorrhagic telangiectasia syndrome as autosomal dominant, no pathogenic variant in trans or cis with NM_005359.6:c.1082G>A (p.Arg361His), and no parent/offspring phase testing, is documented. The required fully penetrant dominant-disease condition is also not established in the case evidence.

Rule
BP2: Observed in trans with a pathogenic variant for a fully penetrant dominant gene/disorder or observed in cis with a pathogenic variant in any inheritance pattern.
Evidence considered
  • The CSPEC raw disease framework lists autosomal dominant inheritance for juvenile polyposis/hereditary hemorrhagic telangiectasia syndrome, but no phase data, pathogenic partner allele, or parent/offspring segregation result is provided for NM_005359.6:c.1082G>A (p.Arg361His).
  • The reviewed variant-specific papers describe functional effects or tumor reporting of p.Arg361His, not a germline cis/trans observation with another pathogenic variant.
Still missing
  • A confirmed pathogenic partner variant is not documented.
  • Parent or offspring testing establishing cis/trans phase is not documented.
  • Full penetrance of the relevant dominant SMAD4 disorder is not established in the available evidence.

BP5 ACMG_2015Not assessed: no affected case is documented with an alternate molecular basis that explains the disease instead of SMAD4 p.Arg361His. Details ▾Hide ▴
Reasoning

BP5 requires the variant to be found in a case with an alternate molecular basis for disease. The available evidence reports SMAD4 p.Arg361His in affected or tumor-derived contexts and functional studies, but does not document an affected case with a different molecular diagnosis that explains the disease instead of this variant.

Rule
Variant found in a case with an alternate molecular basis for disease.
Evidence considered
  • The reviewed variant-specific literature and case evidence do not identify an affected individual carrying SMAD4 p.Arg361His whose phenotype is explained by another pathogenic molecular diagnosis.
Still missing
  • No case with a documented alternate molecular basis for the relevant disease is available.

PVS1 N/ANot applicable: PVS1 requires a null variant (nonsense, frameshift, canonical splice, start-loss or stop-loss), and this is a missense variant. Details ▾Hide ▴
Reasoning

PVS1 (ClinGen SVI PVS1 recommendations, PMC6185798, pvs1_generic_framework) applies only to null variant classes — nonsense, frameshift, canonical ±1/2 splice-consensus, translation-initiation-codon loss, and stop-loss variants — each expected to trigger nonsense-mediated decay or otherwise abolish normal protein function. NM_005359.6:c.1082G>A in SMAD4 is a missense substitution predicted to produce NP_005350.1:p.(Arg361His). As a result, no null-variant mechanism (nonsense-mediated decay, truncation, or canonical splice disruption) is triggered by this variant class, so PVS1's structural prerequisite is not met by this variant class. No VCEP criteria specification covers PVS1 for SMAD4, so the generic ACMG/AMP 2015 definition applies. This determination was made deterministically from the variant's consequence class, prior to LLM adjudication.

PM3 N/ANot applicable: SMAD4-related juvenile polyposis/hereditary hemorrhagic telangiectasia syndrome is documented as autosomal dominant, while PM3 is restricted to recessive disorders. Details ▾Hide ▴
Reasoning

PM3 is not applicable because the available disease framework identifies SMAD4-related juvenile polyposis/hereditary hemorrhagic telangiectasia syndrome as autosomal dominant, whereas PM3 applies to recessive disorders. No recessive SMAD4 interpretation is supported for this case.

Rule
PM3: For recessive disorders, detected in trans with a pathogenic variant
Evidence considered
  • The CSPEC raw disease framework lists the preferred mode of inheritance as autosomal dominant inheritance for juvenile polyposis/hereditary hemorrhagic telangiectasia syndrome; it contains no released criterion-specific rules.

PM4 N/ANot applicable: PM4 requires a protein-length change, and this is a missense variant. Details ▾Hide ▴
Reasoning

PM4 (Richards et al. 2015, PMID:25741868) applies to protein length changes: in-frame insertions/deletions in a non-repetitive region, or stop-loss variants that extend the protein past the reference stop codon. NM_005359.6:c.1082G>A in SMAD4 is a missense substitution predicted to produce NP_005350.1:p.(Arg361His). As a result, no change in protein length occurs, so there is nothing for this criterion to evaluate, so PM4's structural prerequisite is not met by this variant class. No VCEP criteria specification covers PM4 for SMAD4, so the generic ACMG/AMP 2015 definition applies. Reasoning

BP3 (Richards et al. 2015, PMID:25741868) applies to in-frame insertions/deletions located in a repetitive region without a known function. NM_005359.6:c.1082G>A in SMAD4 is a missense substitution predicted to produce NP_005350.1:p.(Arg361His). As a result, the variant does not alter protein length within a repeat region, so there is nothing for this criterion to evaluate, so BP3's structural prerequisite is not met by this variant class. BP7 N/ANot applicable: BP7 requires a synonymous variant, and this is a missense variant. Details ▾Hide ▴
Reasoning

BP7 (Richards et al. 2015, PMID:25741868) is defined for a synonymous (silent) variant for which splicing prediction algorithms predict no impact to the splice consensus sequence and the nucleotide is not highly conserved. NM_005359.6:c.1082G>A in SMAD4 is a missense substitution predicted to produce NP_005350.1:p.(Arg361His). As a result, the encoded protein sequence is altered, so the criterion's silent-variant premise does not hold regardless of any splicing prediction, so BP7's structural prerequisite is not met by this variant class. No VCEP criteria specification covers BP7 for SMAD4, so the generic ACMG/AMP 2015 definition applies. This determination was made deterministically from the variant's consequence class, prior to LLM adjudication.

How the predictions were used
