About U2AF1

U2AF1 encodes a small subunit of the U2 auxiliary factor, a protein complex essential for recognizing pre-mRNA splice sites and removing introns to produce mature messenger RNA. Alterations in U2AF1 are associated with myelodysplastic syndromes, acute myeloid leukemia, and chronic myelomonocytic leukemia. U2AF1 functions as an oncogene in cancer, where recurrent alterations can disrupt RNA splicing and contribute to the development and progression of myeloid malignancies.

Met Not met Not assessed Not applicable

Generic ACMG/AMP rules govern because no U2AF1 VCEP or CSPEC was available.

PS4 was not assessed because the available case frequency lacks a matched unaffected-control comparator and therefore cannot support the required OR >5.0 with a confidence interval excluding 1.0.

PP4 was not met because the reported hematologic phenotypes are not highly specific for a single genetic etiology.

PP5 and BP6 were not met because the exact-variant ClinVar record is single-submitter Oncogenic with no expert-panel Pathogenic/Likely pathogenic or Benign/Likely benign classification.

BP5 was not assessed because the reviewed evidence does not document an alternate molecular basis for disease in a patient carrying U2AF1 c.101C>T.

Generic ACMG/AMP rules govern because no VCEP covers U2AF1.

The missense consequence selects the REVEL path exclusively: REVEL 0.7 supports PP3 at supporting strength, while it does not meet BP4.

Population data support PM2 at Supporting strength: the variant is absent in gnomAD v4.1 and observed once at AF 3.555479705321842e-06 in gnomAD v2.1.

BA1 and BS1 cannot be formally assessed under the required grpmax FAF approach because grpmax FAF is null in both available gnomAD records; their point estimates do not approach the relevant thresholds.

BS2 cannot be assessed from zero homozygotes alone because no qualifying healthy-adult observation or inherited disease penetrance model is documented.

PS3 StrongACMG_2015Met, strong: independent cell, biochemical, minigene, and mouse studies consistently show U2AF1 S34F causes abnormal splicing and related cellular dysfunction. Details ▾Hide ▴
Reasoning

PS3 is met at strong strength. The exact U2AF1 p.Ser34Phe variant was evaluated in multiple independent functional systems. PMID:22158538 used double-reporter and GH1 minigene assays in 293T cells and showed increased splicing efficiency and exon skipping versus wild type. PMID:25311244 demonstrated altered pre-mRNA splicing in primary human CD34+ cells, altered RNA-binding preference in purified U2AF1/U2AF2/SF1 complexes, and concordant GH1/FMR1 minigene effects. PMID:25267526 showed allele-specific changes in 3′ splice-site recognition with mutation-dependent minigene and in vitro splicing effects. PMID:25965570 demonstrated altered hematopoiesis and widespread differential splicing in a doxycycline-inducible human U2AF1(S34F) mouse model. PMID:30054334 showed increased R-loop accumulation, ATR response, DNA damage, apoptosis, and reduced viability in mutant-expressing cells, including primary CD34+ cells. PMID:30842218 used isogenic gene-edited cells and allele-disruption rescue to demonstrate mutant-dependent translational dysregulation and altered cellular responses. Together, the variant-specific, orthogonal assays, primary-cell and in-vivo models, wild-type comparators, and rescue/control designs provide well-established evidence of a damaging functional effect on U2AF1 and its splicing-related product function.

Rule
Well-established in vitro or in vivo functional studies supportive of a damaging effect on the gene or gene product.
Evidence considered
  • Neri et al. 2012, PMID:22158538: U2AF1 p.Ser34Phe was tested in 293T double-reporter and GH1 minigene assays and increased splicing efficiency and exon skipping relative to wild type.
  • Ilagan et al. 2015, PMID:25311244: primary CD34+ RNA-seq, purified-complex RNA-binding assays, and GH1/FMR1 minigenes showed altered splice-site recognition and extensive S34F-associated splicing changes, including 959 of 15,687 genes at the junction level.
  • Ilagan et al. 2015, PMID:25267526: AML transcriptomes, engineered K562 cells, ATR/EPB49 minigenes, and in-vitro splicing assays showed allele-specific altered 3′ splice-site recognition by S34F.
  • Shirai et al. 2015, PMID:25965570: doxycycline-inducible human U2AF1(S34F) mice showed altered hematopoiesis and widespread differential splicing.
  • Chen et al. 2018, PMID:30054334: engineered cell lines and primary CD34+ cells expressing U2AF1S34F showed R-loop accumulation, ATR activation, DNA damage, reduced viability, and apoptosis.
  • Palangat et al. 2019, PMID:30842218: isogenic gene-edited cells, multiple validation lines, and mutant-allele disruption rescue demonstrated S34F-dependent translational dysregulation and altered DNA-damage responses.

PM2 SupportingACMGMet at supporting strength: AF is 3.56e-6 in gnomAD v2.1 and 0 in v4.1, both below the <=0.0001 PM2 threshold. Details ▾Hide ▴
Reasoning

PM2 Supporting is met under the supplied generic threshold because the variant is absent from gnomAD v4.1 (0/18936 alleles) and is present only once in gnomAD v2.1 (1/281256; AF 3.555479705321842e-06), which is below the supplied <=0.0001 threshold. No governing VCEP requires a non-cancer or exome-only subset for this case, and no homozygotes are reported.

Rule
PM2: Absent from controls (or at extremely low frequency if recessive) in Exome Sequencing Project, 1000 Genomes or Exome Aggregation Consortium.
Evidence considered
  • The supplied generic PM2 calibration threshold is <=0.0001 for Supporting strength, citing the ClinGen SVI recommendation and ACMG/AMP source PMID:25741868.
  • gnomAD v2.1 reports 1/281256 total alleles, AF 3.555479705321842e-06, and zero homozygotes.
  • gnomAD v4.1 reports 0/18936 total alleles, AF 0, and zero homozygotes.
  • No VCEP or population-subset specification is present for U2AF1 in this case, so the available all-comers gnomAD records are the default population sources.

PP3 SupportingACMGMet at supporting strength: missense REVEL score 0.7 meets the >=0.644 PP3 threshold. Details ▾Hide ▴
Reasoning

The variant is a missense SNV, NM_006758.3:c.101C>T (NP_006749.1:p.(S34F)), so the governing generic rule requires the missense REVEL path only; splice prediction is not used for PP3/BP4 in this situation. REVEL is 0.7, which meets the generic ClinGen SVI supporting PP3 threshold of >= 0.644 (Pejaver et al. 2022, PMID:36413997). BayesDel is not used because no generic ACMG-strength calibration is established.

Rule
PP3 (supporting): >= 0.644 — ClinGen SVI REVEL calibration (Pejaver et al. 2022, Am J Hum Genet) (PMID:36413997)
Evidence considered
  • Local REVEL v1.3 lookup for NC_000021.8:g.44524456G>A / U2AF1 c.101C>T: REVEL score = 0.7.
  • ClinGen SVI REVEL calibration (Pejaver et al. 2022, PMID:36413997): generic supporting PP3 threshold = >= 0.644.

PS1 ACMG_2015Not met: recurrent p.Ser34Phe reports are somatic, but no previously established pathogenic variant with the same amino-acid change was identified. Details ▾Hide ▴
Reasoning

The variant is NP_006749.1:p.(Ser34Phe). The reviewed literature reports recurrent somatic U2AF1 p.Ser34Phe, but no previously established pathogenic variant with the same amino-acid change was identified. The available ClinVar record is a single-submitter somatic oncogenic record, not an ACMG pathogenic expert-panel determination, so it does not establish PS1.

Rule
Same amino acid change as a previously established pathogenic variant regardless of nucleotide change.
Evidence considered
  • PMID:22158538 reports p.Ser34Phe in 11 of 150 individuals with de novo myelodysplastic syndrome and p.Ser34Tyr in 2 of 150, describing recurrent somatic mutations rather than an ACMG-established pathogenic variant.
  • ClinVar variation 4539541 is classified Oncogenic with criteria provided by a single submitter and somatic origin; no expert-panel pathogenic submission was available.
Sources
PM5 ACMG_2015Not met: p.Ser34Tyr is an alternate missense at residue 34, but its pathogenic status is not established under ACMG/AMP. Details ▾Hide ▴
Reasoning

PMID:22158538 reports the alternate missense change p.Ser34Tyr at the same residue as p.Ser34Phe. However, the paper describes both as recurrent somatic mutations and does not establish p.Ser34Tyr as a pathogenic variant under ACMG/AMP. Therefore, the required pathogenic same-residue comparator is not demonstrated.

Rule
Novel missense change at an amino acid residue where a different missense change determined to be pathogenic has been seen before.
Evidence considered
  • PMID:22158538 reports p.Ser34Tyr in 2 of 150 individuals and p.Ser34Phe in 11 of 150, but characterizes these as recurrent somatic mutations rather than ACMG-established pathogenic variants.
  • PMID:25965570 independently reports four S34F and two S34Y somatic samples, without an ACMG pathogenic determination for S34Y.
Sources
PP4 ACMG_2015Not met: the reported myelodysplastic syndrome phenotype is not highly specific for a disease with a single genetic etiology. Details ▾Hide ▴
Reasoning

PP4 requires a patient phenotype or family history highly specific for a disease with a single genetic etiology. The available case evidence concerns myelodysplastic syndrome and related hematologic malignancy contexts, which are not a highly specific phenotype with a single genetic etiology for U2AF1 c.101C>T. No disease-specific phenotype or family-history evidence meeting PP4 was provided.

Rule
Patient’s phenotype or family history is highly specific for a disease with a single genetic etiology.
Evidence considered
  • PMID:22158538: the variant was studied in individuals with de novo myelodysplastic syndrome, a heterogeneous hematologic disease rather than a phenotype specific to one genetic etiology.
  • PMID:40189296: the patient had dyskeratosis congenita with pulmonary fibrosis and myelodysplastic syndrome, but this is not a phenotype specific to U2AF1 c.101C>T; the paper identifies a germline DKC1 variant and a somatic U2AF1 c.C101T variant.

PP5 ACMG_2015Not met: ClinVar shows single-submitter Oncogenic and no exact-variant expert-panel Pathogenic or Likely pathogenic classification is available. Details ▾Hide ▴
Reasoning

PP5 requires a reputable source to report the variant as pathogenic when the evidence is unavailable for independent evaluation. The exact-variant ClinVar record is classified Oncogenic with criteria provided by a single submitter, not an expert panel, and the available audit contains no expert-panel Pathogenic or Likely pathogenic classification. OncoKB reports Likely Oncogenic, not an ACMG expert-panel Pathogenic/Likely pathogenic classification. Under the specified generic rule, PP5 is not met.

Rule
Reputable source recently reports variant as pathogenic, but the evidence is not available to the laboratory to perform an independent evaluation.

BS3 ACMG_2015Not met: six variant-specific studies report abnormal splicing or cellular dysfunction, with no validated normal-function assay for U2AF1 S34F. Details ▾Hide ▴
Reasoning

BS3 is not met. The reviewed variant-specific functional studies do not show no damaging effect on protein function or splicing; instead, they consistently report abnormal splicing, altered splice-site recognition, altered hematopoiesis, R-loop-associated cellular stress, and mutant-dependent translational effects for U2AF1 S34F. No validated assay demonstrating preserved U2AF1 function or normal splicing for this variant was identified.

Rule
Well-established in vitro or in vivo functional studies show no damaging effect on protein function or splicing.
Evidence considered
  • The reviewed variant-specific studies consistently report damaging or abnormal functional effects rather than preserved function: altered splicing and exon skipping (PMID:22158538), altered splice-site recognition (PMID:25267526 and PMID:25311244), altered hematopoiesis and splicing in vivo (PMID:25965570), R-loop-associated stress and loss of viability (PMID:30054334), and mutant-dependent translational dysregulation (PMID:30842218).

BP1 ACMG_2015Not met: the documented U2AF1 disease-associated pattern is recurrent missense substitution at S34 and Q157, not primarily truncating variation. Details ▾Hide ▴
Reasoning

The available gene-level evidence emphasizes recurrent U2AF1 missense substitutions at S34 and Q157 in hematologic malignancies, with functional studies of S34F altering splice-site recognition and splicing. This does not support a gene for which primarily truncating variants are known to cause disease, so BP1 is not met.

Rule
Missense variant in a gene for which primarily truncating variants are known to cause disease.
Evidence considered
  • PMID:22158538 identifies recurrent U2AF1 S34 missense substitutions and reports functional effects of p.Ser34Phe.
  • PMID:25267526 describes recurrent S34F/S34Y and Q157P mutations and their altered splice-site recognition.
  • PMID:25311244 reports U2AF1 S34F and Q157 substitutions producing altered pre-mRNA splicing, supporting a recurrent missense mechanism rather than a primarily truncating pattern.
Sources
BP4 ACMGNot met: missense REVEL score 0.7 exceeds the <=0.29 BP4 supporting threshold. Details ▾Hide ▴
Reasoning

The variant is a missense SNV, so the governing generic rule requires the missense REVEL path only; the SpliceAI result cannot be substituted for an indeterminate missense REVEL result. REVEL is 0.7, which is above the generic BP4 supporting cutoff of <= 0.29 and therefore does not support BP4. BayesDel is not used because no generic ACMG-strength calibration is established.

Rule
BP4 (supporting): <= 0.29 — ClinGen SVI REVEL calibration (Pejaver et al. 2022, Am J Hum Genet) (PMID:36413997)
Evidence considered
  • Local REVEL v1.3 lookup for NC_000021.8:g.44524456G>A / U2AF1 c.101C>T: REVEL score = 0.7; this is outside the BP4 supporting range of <= 0.29.
  • The available SpliceAI max delta score = 0.002 was not used because this is a missense variant and the governing rule selects REVEL only.
  • ClinGen SVI REVEL calibration (Pejaver et al. 2022, PMID:36413997): generic supporting BP4 cutoff = <= 0.29.

BP6 requires a reputable source to report the variant as benign when the evidence is unavailable for independent evaluation. The exact-variant ClinVar record is Oncogenic with criteria provided by a single submitter, not an expert panel; no exact-variant ClinVar expert-panel Benign or Likely benign classification is available. Therefore BP6 is not met.

Rule
Reputable source recently reports variant as benign, but the evidence is not available to the laboratory to perform an independent evaluation.

PS2 ACMG_2015Not assessed: no documented affected proband with confirmed maternity and paternity, and no documented absence of family history. Details ▾Hide ▴
Reasoning

No case-level evidence documents that U2AF1 c.101C>T (p.Ser34Phe) occurred de novo in an affected proband. The available records provide no maternity confirmation, paternity confirmation, or family-history assessment, so the PS2 definition cannot be evaluated.

Rule
De novo (both maternity and paternity confirmed) in a patient with the disease and no family history.
Evidence considered
  • The case summary and available evidence records identify U2AF1 c.101C>T (p.Ser34Phe) and functional/somatic literature, but contain no proband-parent genotype comparison or documented de novo assessment.
  • The reviewed variant-specific publications describe somatic or functional studies and do not provide the required maternity-confirmed and paternity-confirmed de novo evidence for this proband.
Still missing

PS4 ACMG_2015Not assessed: 11/150 affected individuals were reported, but no matched unaffected-control count supports the required OR >5.0 with a 95% CI excluding 1.0. Details ▾Hide ▴
Reasoning

ACMG/AMP PS4 requires significantly increased prevalence in affected individuals compared with controls; the governing note requires RR or OR >5.0 with a confidence interval that does not include 1.0. PMID:22158538 reports U2AF1 p.Ser34Phe in 11 of 150 individuals with de novo MDS, but it does not report unaffected controls. The available gnomAD observations are population data without a matched case ancestry or a comparable somatic-case design, so a valid case-control OR and 95% CI cannot be established. The literature_pass PS4-indexed PMID:34131312 is a general somatic-variant standards paper whose available abstract does not provide variant-specific case-control counts for U2AF1 c.101C>T. Therefore PS4 is not assessed rather than inferred from the case-only frequency.

Rule
The prevalence of the variant in affected individuals is significantly increased compared to the prevalence in controls. ... Relative risk (RR) or odds ratio (OR), as obtained from case-control studies, is >5.0 and the confidence interval around the estimate of RR or OR does not include 1.0.
Evidence considered
  • PMID:22158538, Results/Figure 3/Table 1: U2AF1 p.Ser34Phe occurred in 11 of 150 individuals with de novo MDS; the paper reports no unaffected or population control cohort for this variant.
  • gnomAD v2.1, source data: 1/281256 alleles overall (AF 3.55548e-06), with 1/24914 African/African American alleles; gnomAD v4.1 reports 0/18936 alleles. These population observations are not a matched-ancestry somatic case-control comparator for the MDS cohort.
  • PMID:34131312, available abstract: general standards for somatic-variant interpretation; no variant-specific U2AF1 c.101C>T case-control counts were reported in the available text.
Still missing
  • A matched-ancestry unaffected-control frequency or a case-control cohort with analyzable counts is required to calculate a valid OR and 95% CI.
  • The PS4-indexed PMID:34131312 abstract does not provide variant-specific U2AF1 c.101C>T case-control data.

PM1 ACMG_2015Not assessed: residue 34 lies in the first zinc finger, but domain-wide absence of benign variation and an allowed hotspot route are not established. Details ▾Hide ▴
Reasoning

The variant is at residue 34, which literature places in the first zinc finger of U2AF1 and a paper describes as a functionally relevant region. However, no VCEP domain table is available, no complete approved critical-domain list exists for U2AF1, and the available evidence does not establish absence of benign variation throughout that domain. The case also reports a statistically significant cancer hotspot, but the governing instructions restrict use of a somatic hotspot toward PM1 to a VCEP-named hotspot route; no VCEP covers U2AF1.

Rule
Located in a mutational hot spot and/or critical and well-established functional domain (e.g. active site of an enzyme) without benign variation.
Evidence considered
  • PMID:25267526 states that U2AF1 S34F and S34Y affect the first zinc finger and reports altered 3′ splice-site recognition, supporting functional relevance of the region but not by itself the PM1 requirement of a critical domain without benign variation.
  • gnomAD v2.1 reports one allele among 281,256 total alleles for the target variant and gnomAD v4.1 reports zero among 18,936 alleles; these data do not provide a domain-wide benign-variation assessment.
  • Cancer Hotspots reports U2AF1 S34 as a statistically significant hotspot, but the applicable instructions do not permit this somatic hotspot route without a VCEP naming it.
Still missing
  • A complete, authoritative U2AF1 critical-domain definition with benign-variation assessment is unavailable.
  • No applicable VCEP hotspot route is available for the reported somatic hotspot.

PM3 ACMG_2015Not assessed: no recessive disease context or parent/offspring phase data establish U2AF1 p.S34F in trans with a pathogenic variant. Details ▾Hide ▴
Reasoning

PM3 cannot be assessed because the available evidence concerns the somatic U2AF1 p.S34F cancer-associated variant and provides no recessive disease context, affected-proband observation, pathogenic variant in the same individual, or parent/offspring testing establishing phase in trans. The available studies describe functional assays and somatic cancer cohorts rather than germline compound-heterozygous or biallelic disease observations.

Rule
For recessive disorders, detected in trans with a pathogenic variant
Evidence considered
  • PMID:22158538 reports recurrent somatic U2AF1 p.Ser34Phe in de novo MDS and functional assays, but does not establish germline phase or a second pathogenic allele.
  • PMID:25267526 reports U2AF1 S34F in AML transcriptomes and engineered cells and explicitly states that the study did not report segregation or individual clinical case data.
  • PMID:25965570 reports U2AF1 S34F in AML samples and transgenic mice, not parent/offspring phase or recessive affected-proband observations.
  • PMID:30054334 reports functional studies of U2AF1 S34F and does not provide patient-level segregation data.
  • PMID:30842218 reports heterozygous U2AF1-S34F cell models and states that the paper does not provide clinical case or segregation data.
Still missing
  • Determine whether U2AF1 is being evaluated for a recessive germline disorder in this case.
  • Obtain affected-proband genotype data showing a second pathogenic U2AF1 variant.
  • Obtain parental or offspring testing that establishes the variants are in trans.

PM6 ACMG_2015Not assessed: no documented affected proband with an assumed de novo occurrence or available parental testing results. Details ▾Hide ▴
Reasoning

No case-level evidence documents an assumed de novo occurrence of U2AF1 c.101C>T (p.Ser34Phe) in an affected proband. There is no reported proband-parent testing or clinical family-history evidence from which an assumed de novo event could be assessed.

Rule
Assumed de novo, but without confirmation of paternity and maternity.
Evidence considered
  • The available case records contain no proband-parent segregation data and no clinical statement that the variant was assumed de novo without maternity and paternity confirmation.
  • The reviewed variant-specific publications do not report a clinical de novo observation for this case.
Still missing
  • Affected proband phenotype and diagnostic context
  • Parental genotype results or documented absence of parental testing
  • Family-history information
PP1 ACMG_2015Not assessed: no affected relatives or informative meioses are documented to demonstrate co-segregation with disease. Details ▾Hide ▴
Reasoning

No affected family members, informative meioses, genotype results, or phenotype-segregation assessments are documented for U2AF1 c.101C>T (p.Ser34Phe). Therefore, co-segregation with disease and any possible increased strength from multiple affected relatives cannot be assessed.

Rule
Co-segregation with disease in multiple affected family members in a gene definitively known to cause the disease. ... May be used as stronger evidence with increasing segregation data.
Evidence considered
  • The case evidence contains no pedigree, affected-relative genotype results, phenotype concordance assessment, or informative meiosis count for this variant.
  • The reviewed publications are functional or somatic cohort studies and do not report familial co-segregation for U2AF1 c.101C>T (p.Ser34Phe).
Still missing
  • Pedigree with affected and unaffected relatives
  • Genotype results for informative family members
  • Phenotype consistency and diagnostic ascertainment for relatives
  • Number of informative meioses
PP2 ACMG_2015Not assessed: recurrent U2AF1 missense mutations are documented, but no validated gene-wide benign-missense rate and mechanism assessment is available. Details ▾Hide ▴
Reasoning

The available sources document recurrent U2AF1 missense mutations, especially at S34, and functional consequences of S34F, but they do not establish both required gene-level conditions: a low rate of benign missense variation and missense variants as a common mechanism of disease in the applicable germline disease context. No validated gene-wide missense constraint or benign-missense-rate threshold is supplied.

Rule
Missense variant in a gene that has a low rate of benign missense variation and where missense variants are a common mechanism of disease.

BA1 ACMG_2015Not assessed: grpmax FAF is unavailable, while observed AF is 3.56e-6 in gnomAD v2.1 and 0 in v4.1 versus the 5% BA1 threshold. Details ▾Hide ▴
Reasoning

BA1 is not assessed because the required filtering metric, grpmax FAF, is null in both available gnomAD records. The available all-comers observations are far below the 5% threshold (gnomAD v2.1: 1/281256 alleles, AF 3.55548e-06; gnomAD v4.1: 0/18936), but the mandated FAF-based rule cannot be formally applied from these records. No non-cancer or exome-only governing specification is present, and no population frequency approaches the stand-alone benign threshold.

Rule
Allele frequency is above 5% in Exome Sequencing Project, 1000 Genomes or Exome Aggregation Consortium.
Evidence considered
  • Generic ACMG/AMP BA1 threshold supplied for this case: allele frequency >=0.05 (5%), with ACMG/AMP 2015 cited in the supplied calibration block (PMID:25741868).
  • gnomAD v2.1 reports 1/281256 total alleles, AF 3.555479705321842e-06, zero homozygotes, and grpmax FAF=null.
  • gnomAD v4.1 reports 0/18936 total alleles, AF 0, zero homozygotes, and grpmax FAF=null.
  • The highest subgroup point estimate in gnomAD v2.1 is 4.013807497792406e-05 in African/African American individuals with 1/24914 alleles; this point estimate is not used as a substitute for grpmax FAF.
Still missing
  • A gnomAD filtering allele frequency (grpmax FAF) for this variant is not reported in the available gnomAD v2.1 or v4.1 evidence.
Sources
BS1 ACMG_2015Not assessed: grpmax FAF and disorder-specific maximum credible frequency are unavailable; observed AF is 3.56e-6 versus the 1% generic BS1 default. Details ▾Hide ▴
Reasoning

BS1 is not assessed because the required filtering metric, grpmax FAF, is null in both available gnomAD records, and no disorder-specific maximum credible allele frequency can be calculated from the supplied evidence because prevalence, penetrance, and allelic heterogeneity for an inherited U2AF1 disorder are not established. The observed all-comers frequencies (3.55548e-06 in gnomAD v2.1 and 0 in gnomAD v4.1) are far below the generic 1% default, but the mandated FAF-based frequency assessment and disorder-specific comparison remain unavailable. The African/African American point estimate is only 1/24914 alleles and is not used as a substitute for grpmax FAF.

Rule
Allele frequency is greater than expected for the disorder.
Evidence considered
  • Generic SVI BS1 default supplied for this case: allele frequency >=0.01 (1%); the prompt identifies Whiffin et al. 2017 (PMID:28518168) as the preferred source for a disorder-specific maximum credible allele-frequency derivation.
  • gnomAD v2.1 reports total AF 3.555479705321842e-06 from 1/281256 alleles, zero homozygotes, and grpmax FAF=null.
  • gnomAD v4.1 reports total AF 0 from 0/18936 alleles, zero homozygotes, and grpmax FAF=null.
  • The gnomAD v2.1 African/African American subgroup has a point estimate of 4.013807497792406e-05 from 1/24914 alleles; this does not replace the required grpmax FAF.
Still missing
  • A gnomAD filtering allele frequency (grpmax FAF) is not reported for the variant.
  • No inherited U2AF1 disorder prevalence, penetrance, or allelic-heterogeneity inputs are available for a maximum credible allele-frequency calculation.

BS2 ACMG_2015Not assessed: gnomAD shows 0 homozygotes, but no documented healthy adult with qualifying zygosity, age, phenotype, and early-onset full-penetrance disease context exists. Details ▾Hide ▴
Reasoning

BS2 is not assessed. gnomAD reports zero homozygotes in v2.1 and v4.1, but population counts do not document a healthy adult individual carrying the variant, the relevant zygosity, age, phenotype, or full penetrance of an early-onset disorder. The supplied case evidence concerns a somatic U2AF1 cancer variant and does not establish an inherited disease model or qualifying healthy-adult observation.

Rule
Observed in a healthy adult individual for a recessive (homozygous), dominant (heterozygous), or X-linked (hemizygous) disorder, with full penetrance expected at an early age.
Evidence considered
  • gnomAD v2.1 reports zero homozygotes for 1/281256 total alleles.
  • gnomAD v4.1 reports zero homozygotes for 0/18936 total alleles.
  • Neither population record identifies individual carrier age, health status, phenotype, zygosity for the relevant disorder, or penetrance; absence of homozygotes alone is not a documented healthy-adult observation.
Still missing
  • No documented healthy adult carrier with sufficient age, phenotype, zygosity, and disease-model information is available.
  • The inherited disease mode and expected age-related penetrance needed to apply BS2 are not established.

BS4 ACMG_2015Not assessed: no affected relatives with documented variant-negative testing are available to demonstrate lack of segregation. Details ▾Hide ▴
Reasoning

No affected family members carrying or lacking U2AF1 c.101C>T (p.Ser34Phe) are documented, and no phenotype-confirmed non-segregating relatives are described. Lack of segregation therefore cannot be established or distinguished from phenocopy, incomplete ascertainment, or another familial cause.

Rule
Lack of segregation in affected members of a family.
Evidence considered
  • The available case records contain no affected-relative testing or phenotype-confirmed non-segregation data for U2AF1 c.101C>T (p.Ser34Phe).
  • The reviewed publications do not provide family-based non-segregation evidence for this variant.
Still missing
  • Affected relatives with reliable phenotype assessment
  • Variant testing results in affected relatives
  • Family structure and ascertainment sufficient to evaluate non-segregation
  • Assessment of phenocopy or additional pathogenic variants
BP2 ACMG_2015Not assessed: no affected-proband or family phase data show U2AF1 p.S34F in trans or cis with a pathogenic variant. Details ▾Hide ▴
Reasoning

BP2 cannot be assessed because no affected-proband or family genotype data establish U2AF1 p.S34F in trans with a pathogenic variant in a fully penetrant dominant disorder, or in cis with a pathogenic variant under any inheritance pattern. The available evidence is from somatic cancer cohorts and functional cell studies and does not provide phase information.

Rule
Observed in trans with a pathogenic variant for a fully penetrant dominant gene/disorder or observed in cis with a pathogenic variant in any inheritance pattern.
Evidence considered
  • PMID:22158538 describes recurrent somatic U2AF1 p.Ser34Phe in MDS and functional assays, without cis/trans testing with a pathogenic variant.
  • PMID:25267526 evaluates U2AF1 S34F in AML transcriptomes and engineered cells and reports no segregation or individual clinical case data.
  • PMID:25965570 evaluates U2AF1 S34F in AML samples and transgenic mice, without family phase data.
  • PMID:30054334 evaluates U2AF1 S34F in engineered cell lines and primary hematopoietic cells, without patient-level phase data.
  • PMID:30842218 uses heterozygous U2AF1-S34F cell models and reports no clinical case or segregation data.
Still missing
  • Obtain genotype data from an affected proband or family showing a second pathogenic variant.
  • Establish phase as trans for a fully penetrant dominant disorder or cis for any inheritance pattern.
  • Clarify the relevant germline inheritance model and whether the disorder is fully penetrant and dominant.

BP5 ACMG_2015Not assessed: the available case reports do not document an alternate molecular diagnosis explaining disease in a patient carrying U2AF1 p.Ser34Phe. Details ▾Hide ▴
Reasoning

BP5 requires the variant to be found in a case with an alternate molecular basis for disease. The reviewed evidence establishes that U2AF1 p.Ser34Phe occurs in MDS and provides functional data, but it does not document an individual case in which a separate molecular diagnosis explains the disease. BP5 is not assessed because the required alternate molecular basis is not available in the cited evidence.

Rule
Variant found in a case with an alternate molecular basis for disease.
Evidence considered
  • PMID:22158538: reports U2AF1 p.Ser34Phe in de novo MDS but does not document an alternate molecular basis for disease in the reported cases.

  • A patient-level report documenting U2AF1 c.101C>T together with an independent molecular diagnosis explaining the same disease is required.

PVS1 N/ANot applicable: PVS1 requires a null variant (nonsense, frameshift, canonical splice, start-loss or stop-loss), and this is a missense variant. Details ▾Hide ▴
Reasoning

PVS1 (ClinGen SVI PVS1 recommendations, PMC6185798, pvs1_generic_framework) applies only to null variant classes — nonsense, frameshift, canonical ±1/2 splice-consensus, translation-initiation-codon loss, and stop-loss variants — each expected to trigger nonsense-mediated decay or otherwise abolish normal protein function. NM_006758.3:c.101C>T in U2AF1 is a missense substitution predicted to produce NP_006749.1:p.(Ser34Phe). As a result, no null-variant mechanism (nonsense-mediated decay, truncation, or canonical splice disruption) is triggered by this variant class, so PVS1's structural prerequisite is not met by this variant class. No VCEP criteria specification covers PVS1 for U2AF1, so the generic ACMG/AMP 2015 definition applies. This determination was made deterministically from the variant's consequence class, prior to LLM adjudication.

PM4 N/ANot applicable: PM4 requires a protein-length change, and this is a missense variant. Details ▾Hide ▴
Reasoning

PM4 (Richards et al. 2015, PMID:25741868) applies to protein length changes: in-frame insertions/deletions in a non-repetitive region, or stop-loss variants that extend the protein past the reference stop codon. NM_006758.3:c.101C>T in U2AF1 is a missense substitution predicted to produce NP_006749.1:p.(Ser34Phe). As a result, no change in protein length occurs, so there is nothing for this criterion to evaluate, so PM4's structural prerequisite is not met by this variant class. No VCEP criteria specification covers PM4 for U2AF1, so the generic ACMG/AMP 2015 definition applies. Reasoning

BP3 (Richards et al. 2015, PMID:25741868) applies to in-frame insertions/deletions located in a repetitive region without a known function. NM_006758.3:c.101C>T in U2AF1 is a missense substitution predicted to produce NP_006749.1:p.(Ser34Phe). As a result, the variant does not alter protein length within a repeat region, so there is nothing for this criterion to evaluate, so BP3's structural prerequisite is not met by this variant class. BP7 N/ANot applicable: BP7 requires a synonymous variant, and this is a missense variant. Details ▾Hide ▴
Reasoning

BP7 (Richards et al. 2015, PMID:25741868) is defined for a synonymous (silent) variant for which splicing prediction algorithms predict no impact to the splice consensus sequence and the nucleotide is not highly conserved. NM_006758.3:c.101C>T in U2AF1 is a missense substitution predicted to produce NP_006749.1:p.(Ser34Phe). As a result, the encoded protein sequence is altered, so the criterion's silent-variant premise does not hold regardless of any splicing prediction, so BP7's structural prerequisite is not met by this variant class.

  • Local REVEL v1.3 lookup for NC_000021.8:g.44524456G>A / U2AF1 c.101C>T: REVEL score = 0.7; this is outside the BP4 supporting range of <= 0.29.
  • The available SpliceAI max delta score = 0.002 was not used because this is a missense variant and the governing rule selects REVEL only.
  • ClinGen SVI REVEL calibration (Pejaver et al. 2022, PMID:36413997): generic supporting BP4 cutoff = <= 0.29.
